CORRECTION IN REGISTRATION LISTING
(For Delegates Only)
It is important that the delegate lists published after the convention in the Convention Proceedings  
be as accurate as possible. Prior to and during the convention, and within two days of the close of 
convention, please visit the link below to complete FORM XX to report any corrections to your 
personal listing in the front of this workbook.

While the convention is in session only, you may use the lower section of this page as your notice to 
the Office of the Secretary to report corrections. Remove it from your Workbook and deliver it to the 
Today’s Business office or hand it to the Secretary at the convention:

(Please note that this form is not to be used when there is a change of delegates. In such cases, 
necessary documentation is required from the Secretary of the District.)
